  ?Product Monographs can be used as a resource and reference, however AHS recommendations for influenza immunization are based on the Â鶹ӳ»­ Immunization Policy.   • For use at Â鶹ӳ»­ (AHS) immunization programs. Use these forms when a parent or alternate decision-maker is not able to be with the person being immunized at an AHS immunization service. An alternate decision-maker could be an agent, guardian, specific decision-maker or co-decision-maker.
    • The parent or alternate decision-maker should complete these forms and send it with the person being immunized, to the AHS immunization service.
    • For alternate decision-makers ¨C please also send a copy of documents to show that you are authorized to be the alternate decision-maker.

• Influenza Immunization Record
  The Influenza Immunization Record is to be used for internal purposes only (e.g. Data entry, partner reporting). It is not intended to be used as a record of immunization and is not to be given to the client by the immunizer. For the fillable online form ¨C complete online and print copies as needed or print and complete by hand and photocopy as needed.
