Home Care

Provides medical support for people so they can live in their own homes

Home Care helps people who live at home and need support such as:

  • Care after surgery
  • Long-term care
  • Palliative care
  • Respite services

A case manager, who is usually a nurse, will meet with the clients and help decide what services will best support them.

licensed practical nurses (LPNs), occupational therapists (OTs), registered nurses (RNs)
Service Access

Healthcare providers should consult the for service referral information.

Referrals to Home Care are placed though Continuing Care Access (CCA). The CCA team can be contacted by you, family, friends, healthcare providers or community agencies to request services.


Fees

Most services are covered for people with 鶹ӳ Health Care Insurance coverage, but there may be charges for services not covered by 鶹ӳ Health Care Insurance. Out of province clients looking for home care services in 鶹ӳ should inquire on any potential charges prior to obtaining services.
