Provides Social Work support.Assist patients and address family concerns regarding: financial assessment and Interventions to assist patient and family in problem-solving and planning for the financial elements family and caregiver coping adjustment support mediate and assist with conflict resolution and provide advocacy to support patient and family goals decision making capacity process, alternate decision makers and related legal documents identify and connect patients with appropriate community supports and resources
