Access & Disclosure / Health Information Management

Releases individually identifiable health information to patients, authorized representatives, service providers and other third parties in accordance with the Health Information Act.

In accordance with ΒιΆΉΣ³»­'s Health Information Act, this is a service which enables the release of individually identifiable health information with or without the consent of an individual.

ΒιΆΉΣ³»­ (Hospital-inpatient / outpatient) does not maintain records for private physicians, medical clinics, or walk-in centres, please contact them directly to attempt to obtain a copy of their records.

Healthcare providers can submit a request for ΒιΆΉΣ³»­ records by contacting our office via fax or email.
