Transition service for teenage clients with diabetes, previously followed by the Â鶹ӳ»­ Children's Hospital Diabetes Clinic.

This service helps teenage clients to transition to the adult health care system when they have been previously followed through the Â鶹ӳ»­ Children's Hospital Diabetes Clinic.
